My lost or damaged lottery ticket is registered to a player card
Contact us on 131 868 if you have lost or damaged your lottery ticket that was purchased in-store with your player card. Our team will be able to authenticate your details and provide relevant information about the lost ticket.
We can also place a ‘Stop Payment’ on your lost or damaged ticket. This will prevent any other person from attempting to claim your lottery prize, provided it has not yet been paid.
My lost or damaged lottery ticket is unregistered
If your lost or damaged lottery ticket is not registered to a player card, you will need to complete a Lost or Damaged Ticket Search Form. This information will assist us in locating the ticket in our lottery system.
Depending on where the ticket was purchased, download and print the relevant form here. If multiple tickets are lost or damaged, a form will need to be completed for each ticket.
Once complete, forward the Lost or Damaged Ticket Search Form to the postal address listed on the form, and include the original ticket (if applicable). We recommend you keep a copy as well.
Completion of the Statutory Declaration section is a mandatory requirement.
To maintain the integrity of our claims process, we can only process limited Lost/Damaged Ticket Search requests for each ticket. This helps prevent misuse and ensures fair outcomes for all players. As such, we may decline to accept repeated requests relating to the same ticket.
Successful claims resulting in a prize will be paid pending verification and approval as per the Condition of Search listed on the form.
