A Lost or Damaged Ticket Search Form will need to be completed for lost Instant Scratch-Its tickets.
This form will assist in locating the lost ticket in our lottery system.
For multiple lost Instant Scratch-Its tickets, a form will need to be completed for each ticket.
Download and print the Lost or Damaged Ticket Search Form according to where the ticket was purchased:

Please forward the completed Lost or Damaged Ticket Search Form to the relevant address listed on the form.
A search fee of $15 ($22 for South Australian customers) must be paid either by debit card or Visa credit card prior to the search proceeding.
Completion of the Statutory Declaration section in the form is a mandatory requirement.
Successful claims will be paid after the following periods:
- In Victoria, Tasmania, Northern Territory and Queensland, the prize will be paid four weeks from the date of Instant Scratch-Its ticket purchase.
- In New South Wales and the Australian Capital Territory, the prize the prize will be paid eight weeks from the date of Instant Scratch-Its ticket purchase.
- In South Australia, the prize will be paid 12 months from the date of Instant Scratch-Its ticket purchase.
